myNurse is a digital health program designed to help older adults with chronic conditions live happier, healthier, more independent lives. Through a combination of remote health monitoring and one-on-one coaching, myNurse helps members build healthier habits and manage chronic conditions like diabetes and high blood pressure, one day at a time. Balancing technology with a human touch, myNurse leverages an augmented intelligence platform that analyzes historical patient outcomes and interactions to empower care providers with the words, phrases, resources and approaches most likely to spark behavior change that lasts and leads to long-term health improvement. myNurse members have shown a 12mm Hg average reduction in systolic blood pressure (a 20% improvement over the typical monotherapy hypertensive drug), 60% of members with diabetes have had a reduction in average blood glucose readings, and 64% of members have had a reduction in critical alerts as compared to baseline.
