Catalyst Health Network improves health, coordinates care, and lowers cost – creating sustainable and predictable value. The network is comprised solely of primary care physician members and managed by StratiFi Health, a physician services, and population health organization that provides integrated solutions to support and optimize the Physician-Patient relationship and proactively shape the patient's healthcare experience. Providers are powered with a team-based care approach that guides the patient to ensure they receive the appropriate care in the appropriate setting for all their healthcare needs. Our physician business services empower independent practices with insights, tools, and actionable strategy, which is essential for successful transformation and longevity. We directly touch the lives of millions by improving their health while also decreasing overall health care costs.
